6th, 7th, & 8th Grade Nobel Charter Middle School Elective Google Forms for 2024 – 2025
The elective forms below are for new Nobel Middle School students only.
1. You can only submit the form once! You will not be able to edit your form once submitted.
2. You will need to use your LAUSD Mymail account to sign in. This is the same email and password you use to log in you use for your classes on Schoology. (No private email addresses) If someone else (Parent or a sibling) is already logged in on the device under a different Gmail account you will need to log out and switch Google accounts using your student's Mymail Google sign in. If you complete the form on your student's Chromebook you will not have this access issue.
3. If your student is new to LAUSD use the same email address that is on the registration form.

4. You will need your student ID# (Located on your Student ID Card OR Report Card. It is a 10 character ID and the 6th character will always be an F, M, or X. Examples - 061408F024 or 092808M037 or 112108X115). If your student is new to LAUSD and you do not have a student ID number please use your students birth date (month, day, and year - example - 04/16/2011).
5. You will need a parent's email address.
7. Please review the elective descriptions page before filling out your form.
8. You will need to choose 5 different preferences to complete and submit the form.
9. Once you submitted your form a copy will be automatically sent to your LAUSD email.
10. Please remember that you are not guaranteed one of your 5 preferences.
